What is your new client procedure?

  • Initial FREE Consultation to discuss goals of the client and family

  • Comprehensive Evaluation including a detailed Plan of Care, client centered goals and initial home exercise plan

  • Treatment sessions scheduled based on Plan of Care determined by clinician and client/family

What types of health insurance do you accept?

We accept Anthem BCBS, Aetna, CIGNA, ConnectiCare, United Health Care, TriCare and Medicaid Husky A/B/C Insurance Policies.

*TriCare: We are a preferred non-network provider

*Husky: We can only see patients 0-21 years old per plan policy

We also offer an affordable Private Pay option for patients without insurance.

Monday: 8:15-7:00

Tuesday: 8:15-7:00

Wednesday: 8:15-7:00

Thursday: 10:30-7:00

Friday: 8:15-3:00

Saturday: CLOSED

Sunday: CLOSED

We book each session 1:1 for 45 minutes

How is Crescent Moon Care different?

At Crescent Moon care all sessions are 1:1 with a Physical Therapist. Sessions are 45 minutes long and there is no overlap between patients. Patients goals and values are incorporated into each specific Plan of Care to ensure the most effective and personalized care possible.
